2. Membership Verification Form – Membership Verification

(Send link to AHEPA or DOP chapter officer, https://www.ahepa-district14.org/local-chapters)  

3. Recommendation Form – Recommendation of Educator Form (Send link to professor/teacher to complete.)

4. Extracurricular Form – Extracurricular Verification Form (Send link to activity supervisor to complete.)

5. Submit supporting documentation (transcript, essay, letter of acceptance for incoming freshman, 2025 federal tax form) by email or mail:
